A challenging condition in Malawi

Infectious diseases are very frequent in Malawi and Africa. This applies not only to typical tropical diseases, but   also infections like TBC and HIV/AIDS  are endemic in Malawi. Working as an orthopaedic surgeon you encounter many cases of neglected infections, leading to disastrous consequences for the patient
I present here a young patient who suffered from chronic pain in the thigh, with swelling  and discharge of pus from a small sinus. Experienced healthcarers will probably easily recognize the condition. In Norway we rarely see presentations like this. I operated the patient and removed the specimen you see on the picture.
What do you think it is?
